What is our process?

• Directly admit your patients saving your patients time from waiting in the ER
• Contact provider directly on their cell
• We will discharge the patient back to you from the acute hospital

• If step down care is needed, we will follow the patient and continue to manage their care
• Once patient is ready for discharge, we ensure the patient is discharged back to your care
• Our goal is to treat your patient and return them to you
